Before starting, please ensure that you have the correct details to hand. If you are updating your address or photograph you should make sure that you have the relevant proof of photograph (or referee's details) and the proof of address required. Full details of proofs required are available at:
www.entitlementcard.org.uk/proofs. You only need to provide proof for changes that you are making.

If you can't fill in this replacement card request form then please visit the Glasgow school where the cardholder is registered or your local library to complete a paper application form. You can find the location of your nearest library by visiting: www.glasgowlife.org.uk/libraries
Please note that we only deal with National Entitlement Cards for children and young people between 5 and 26 living in the Glasgow City Council area. If you live outwith this area then please contact your local council. If you're over 21 then your card will not include a travel entitlement, but you will still be able to access the other benefits of having a Young Scot NEC (including discounts and rewards).
We only deal with travel cards for young people under 22 who do not have an enhanced concessionary travel entitlement. If you are entitled to another type of concessionary travel (including disability travel, or travel with a companion) please contact SPT to discuss your card.
