The Gaff Dance Event is taking place soon. In order to attend the event, you must complete the event consent form by answering the questions below fully. If you're under 18 then the form must be completed by your parent or guardian.

Please try to complete all sections of the form - especially if you're looking for transport from a local high school. Spaces for this are limited and the earlier that you return the form the easier it will be to accommodate your transport needs.

Once you have the form complete then you and your parent will receive a copy by e-mail if you have shared their e-mail with us. You should keep this securely on your device so that we can check it on the night if we need to - you don't want us to miss it and not be able to let you in!
